We want to partner with you to help reduce HIV diagnoses and increase suppression rates across the state of Michigan and our Medicaid population. Here’s how.
Care management services
Our care management team supports your Priority Health Medicaid patients diagnosed with HIV by:
- Offering personalized care management plans that help patients keep track of appointments and medications
- Connecting patients with community health workers so they can stay up to date on community events, local support groups and more
- Helping patients understand the programs and services available to them through their Priority Health Medicaid plan
Transportation services
We never want lack of transportation to keep your patients from getting the care they need. That’s why we offer transportation services to all our Priority Health Medicaid members. Members can schedule a ride by calling 833.944.0535 or visiting priorityhealth.com/needaride to learn more.
Covered therapies to help prevent or treat HIV
Priority Health Medicaid members have access to covered therapies that can help prevent or treat HIV, at little to no cost to them. HIV medications are covered by the Michigan Fee For Service (FFS) Medicaid plan. Members and providers can use the FFS MRx Providers | Drug lookup tool to understand coverage or call the Beneficiary Help Line at 1. 800. 642. 3195.
To help local health agencies connect patients who have a higher risk of contracting HIV to providers that can support their care, current PrEP prescribers are encouraged to register at https://npin.cdc.gov/organization/submit.
