Self-funded plans administered by PHP TPA Services are provided with a unique identification that provides access to Lakeland Care Network providers who are contracted with PHP. Each patient member covered under a TPA plan will be assigned an individual identification card.
Our TPA has advised PHP that they are receiving notifications that some members are experiencing issues when attempting to obtain services from Corwell providers and facilities. It appears that the provider may not be aware that they are in network as the TPA card does look different from the fully insured one.
