PHP of Northern Indiana highly recommends utilizing electronic prior authorization (ePA) as the preferred method for obtaining prior authorization for pharmacy prescriptions. 

For fully insured members under PHP coverage, utilizing ePA ensures the quickest authorization process. 

Our pharmacy benefit management partner, Express Scripts, accepts ePAs from all entities.

 Please remember to attach supporting records when submitting ePA requests.

Medical and specialty drug prior authorization will continue to be reviewed by our authorization vendor; Vãlenz.
