Cost: $20.00 per person, if registered by noon the Friday prior to the meeting
$30.00 per person for late registrants
Payment: Payment is expected by the meeting date and can be made at the door via a check made payable to Lakeland Medical Office Association, or with cash (exact change), or you can mail your check to LMOA, PO Box 144, Stevensville, MI 49127.

Please note: If you are unable to attend and have already sent in your RSVP, but it is past the RSVP deadline, please send someone else in your place as you will be responsible for the cost of the lunch.
