Please help us by reminding your primary care practices that the Blue Care Network Qualification Form must be submitted online within the BCN Health e-Blue℠ application. The Blue Care Network Qualification Form cannot be faxed.
Many BCN patients with coverage through Healthy Blue Living℠ need their qualification forms submitted by the end of March. Some offices are calling Provider Inquiry to request a fax number. Faxing is not an option for the BCN form.
