Blue Cross Blue Shield of Michigan and Blue Care Network have implemented a monthly data feed to inform you of your group’s assigned BCN AdvantageSM members who are eligible for the Landmark Health high-intensity in-home care program for members with multiple chronic conditions. Landmark is offered as a benefit to Blue Cross Blue Shield of Michigan and BCN’s Medicare Advantage members in the lower peninsula who meet specific eligibility criteria. The monthly MCG Landmark Eligibility file will be sent to your group’s mailbox during the third week of each month. Only medical care groups with Landmark eligible members will receive a file. If your group is receiving a file this month you can expect to find it in your mailbox on Tuesday, May 16. These Landmark eligible members will also be included in the MCG eligibility file.
