Priority Health - Medicare and Medicaid Support Programs through 2020

As partners with you in your patient’s health, we want to keep you informed of the various member programs happening now through the end of the year, with the goal to help keep Medicare and Medicaid members, your patients, safe and healthy with more resources and support in addition to their doctor visits. 

 

FIT kit colon cancer screening campaign

We’re sending in-home fecal immunochemical test (FIT) kits to select Medicare members who are past due for a colon cancer screening. A copy of the results will be sent to the member and their primary care provider.

 

BioIQ A1c and microalbumin at-home test kits

We’re partnering with BioIQ and sending at-home A1c kits to a small group of Medicare and Medicaid members with diabetes who have not completed their A1c since Dec. 1, 2019. We’re also sending microalbumin home test kits to select members who have not completed either their A1c or microalbumin tests since 2019. Members will receive a letter or email from BioIQ and Priority Health explaining the purpose and importance of the kit. A copy of the results will be sent to the member and their primary care provider.

 

Signify Health HRE+ pilot

In partnership with Signify Health, we’re offering a small number of Medicare members participation in this free program to identify and address social determinants of health (SDOH). If a member chooses to participate, a social care coordinator will help the member identify community-based resources to support their needs, such as access to food, safe housing, transportation and more. This pilot program is meant to supplement the care and support you give your patients by addressing their SDOH needs. The program is voluntary for the member, and there’s nothing you need to do.

 

Papa

PriorityMedicare EdgeSM and PriorityMedicare IdealSM members have a companionship benefit through our new partnership with Papa. Members can take advantage of up to eight hours per month of daily living assistance with a Papa Pal. Visits can include transportation to the grocery store, pharmacy and/or doctor appointments, light housework and cooking, technology support/lessons and companionship. This service addresses members social determinants of health needs specific to social isolation.

 

MyAdvocate

We partner with Change Healthcare using their My Advocate® service, a leading provider of outreach and advocacy services for Medicare members, helping them save money and improve their overall health and wellness through government and community programs. In addition to education, advocates work with members to complete and submit program applications. Advocates determine if members may be eligible for the Medicare Savings Program (MSP) and Extra Help with Part D Prescription Drug costs and local programs such as:

  • Assistance with utility bills
  • Home repair
  • Caregiver services
  • Nutrition assistance
  • Scheduling doctor appointments
  • Disease education
