TIME:              11:45 am - 2:00 pm (Lunch 11:45 - 12:30, program begins at 12:30) 

LOCATION:      Hilton Garden Inn, 1300 Cinema Way, Benton Harbor, MI

COST:              LMOA Members - $15.00 per person; non-members and late registrants are $30 per person (any rsvp received after noon on 1/13/17 is late)

PAYMENT:       Payment can be made at the door via a check made payable to Lakeland Medical Office Association, or with cash (exact change please), or you can mail your check to LMOA, PO Box 144, Stevensville, MI  49127.
