A case of a high risk surgical patient presenting with biliary obstruction and acute cholangitis with sepsis. The diagnosis and management of acute cholangitis will be discussed with an emphasis of the role of ERCP and cholesystectomy in the treatment. The evaluation of surgical risk will be discussed.
